The agreement between you and VitalisRx.
Who we are
VitalisRx, LLC operates a technology platform and a clinic in Fort Lauderdale, Florida. Clinical services are provided by independently licensed Florida nurse practitioners and registered nurses. The platform does not practise medicine and does not influence clinical judgement.
Eligibility
You must be at least 18 years old, physically located in a state where we are licensed, and able to give your own medical consent. Some treatments carry a higher minimum age, shown on the treatment page.
You agree to give accurate health information and to update it when it changes.
No guarantee of treatment
Your clinician may decline to prescribe. That is a clinical decision and it is final. If you are declined at the first review, we refund your first membership charge in full.
Your account
Keep your password private. Tell us immediately if you think someone else has access. You are responsible for activity under your account.
Termination
You can cancel any time from your account. We may suspend an account for non-payment, for misuse, or where continuing care would be clinically unsafe. We will always give you your records and a reasonable window to find another provider.
Disputes
These terms are governed by Florida law. Nothing here limits any right you have that cannot be limited by law, and nothing here limits liability for a clinician’s professional negligence.

When you accept this at sign-up we store the exact version shown above alongside the date and time. If we publish a new version, we ask you again rather than applying it retroactively.
