What a remote visit can and cannot do, who is treating you, and when you need to be seen in person instead.
What telehealth is
Telehealth means your clinician evaluates and treats you using video, phone or secure messaging instead of being in the same room as you. VitalisRx, LLC provides care through licensed Florida nurse practitioners, with registered nurses providing in-clinic services at VitalisRx Las Olas, 501 E Las Olas Blvd, Suite 300, Fort Lauderdale, FL 33301.
Your nurse practitioner will review your intake, may hold a live video visit with you, and will decide whether prescribing is appropriate. A prescription is never guaranteed. Paying a membership fee buys clinical time and access, not a particular medication.
What telehealth cannot do
Your clinician cannot physically examine you, listen to your heart and lungs, or draw your blood. Some conditions cannot be diagnosed safely without those things, and your clinician may tell you that you need to be seen in person.
This service is not for emergencies. If you have chest pain, trouble breathing, signs of a stroke, a severe allergic reaction, or thoughts of harming yourself, call 911 or go to the nearest emergency department. For a mental health crisis, call or text 988.
We do not prescribe controlled substances of any schedule. If your condition needs one, we will tell you and help you find in-person care.
Risks of remote care
Information you give us may be incomplete or inaccurate, which can lead to a wrong decision. Video or messaging can fail. Electronic transmission, though encrypted, can in rare cases be intercepted. Delays in evaluation are possible.
You may withdraw this consent at any time without affecting your right to future care, though we cannot continue prescribing without it.
Where you must be located
Florida law licenses your clinician to treat patients located in Florida. You must be physically in Florida at the time of your visit. If you move or travel, tell us — we will pause care rather than treat you across a state line we are not licensed in.
Records and follow-up
Everything from your visits becomes part of your medical record and is available to you in your account. We encourage you to have a primary care provider and, with your permission, will send them your records.
