Notice of privacy practices

VitalisRx, LLC is a covered entity under the Health Insurance Portability and Accountability Act. We are required by law to protect the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

For treatment — your nurse practitioner, our registered nurses, and the pharmacy filling your prescription use your record to care for you.

For payment — to bill your membership and to process medication and visit charges through our payment processor.

For health care operations — quality review, clinician auditing, and running the service. Every access to your chart by our staff is logged with who, when and why.

We do not sell your health information. We do not use it for advertising. We do not share it with data brokers.

As required by law, including public health reporting, reporting suspected abuse or neglect, responding to a valid court order, and reporting a serious and imminent threat to your health or someone else’s safety.

To inspect and get a copy of your record, in electronic form, within 30 days. You can download it yourself at any time from Settings in your account, or ask us at records@vitalisrxcare.com.

To request a correction if you believe something is wrong. Write to records@vitalisrxcare.com and tell us what is wrong and what it should say.

To an accounting of certain disclosures we have made. Ask at records@vitalisrxcare.com.

To request that we communicate with you a particular way, or restrict certain disclosures.

To be notified if your unsecured health information is breached.

To complain to us at privacy@vitalisrxcare.com or to the U.S. Department of Health and Human Services Office for Civil Rights. We will never retaliate against you for filing a complaint.

We use third parties for hosting, payments, e-prescribing, video visits, and email. Every vendor that can touch your health information operates under a signed business associate agreement. We do not send text messages at all — carriers will not sign one. Anything clinical stays in the secure message thread inside your account, and email is used only to tell you there is something waiting there.

If you think your information has been exposed, or you have found a weakness in this site, write to security@vitalisrxcare.com. We would rather hear it from you than find out later, and we will not pursue anyone who reports a genuine security problem in good faith.
